Types of Breast Reconstruction

Dr. Rafael Gottenger, Plastic Surgeon with Baptist Health South Florida, explains for breast reconstruction doctors used to place the implant under the muscle, but now they have evolved and they do it above the muscle.

“There’s less pain and they recover faster,” he says. About the implants, he explains an expander is used when a patient does not have the space to put the implant in, and there is another type with a remote control that allows patient to expand the tissue by pushing it with the air from the inside of the implant.

LET’S TALK ABOUT THE DIFFERENT TYPES OF BREAST RECONSTRUCTION, THEY CAN HAVE THAT IMPLANT DONE ABOVE OR UNDER THE MUSCLE, IS THAT RIGHT? > IN THE PAST WE USED THE MUSCLE, THE MORAL MUSCLE HERE, AND WE DID THE RECONSTRUCTION UNDER THE MUSCLE. I HAVE BROUGHT SOME EXAMPLES, AN EXPANDER, WHEN A PATIENT DOES NOT HAVE THE SPACE RHETORIC — RIGHT AWAY, LIKE A PREGNANCY, YOU MAY HAVE A PREGNANCY, WITH A BIGGER BELLY BUT IT THERE IS A SLOW EXPANSION OF THE SKIN TO ACCOMMODATE IT. SO WE HAVE THESE EXPANDERS, SOMETHING LIKE THIS, YOU CAN PUT THIS IN, AND LOCALIZE IT AFTER A MASTECTOMY, AND YOU CAN SLOWLY SPAM IS WITH AN INJECTION WITH SALINE. THERE ARE SOME WITH TWO PORTS, LIKE THIS ONE, ONE THAT WE CAN PUT IN AND ONE THAT WE CAN USE TO REMOVE ANY COLLECTION AROUND THE EXPANDER. >AND YOU JUST — AND THESE ARE NOT ONLY FOR CANCER PATIENTS? > MOSTLY FOR CANCER PATIENTS. AND THE NEWEST ONES ARE THESE, THIS IS A CANISTER INSIDE A RESERVOIR OF CO2, THE PATIENT DOES THIS AT HOME, THEY LOCALIZED WHERE IT IS, ONCE THEY FIND IT THEY PUSH AIR AND THEY PUT THE AIR INSIDE. THEY CAN DO IT AT HOME, THEY CAN BE IN ORLANDO, MIAMI, THEY CALL ONCE A WEEK TO CHECK ON HOW IT’S DOING BECAUSE WE CAN CONTROL THE AMOUNT EVERY TIME THEY PUT IN WITH A REMOTE CONTROL.
